Students should be the ones accountable for their learning.  There should be no surprises for anyone.  
Encourage students to keep these three questions in mind always.  
1. Where do I stand know?
2. Where do I need to be?
3. How can I get there?

Assess your students and see what they know about their learning.  I was surprised with my results!!

Introduce S.M.A.R.T goals to your students, and get them in the habit of forming them often.  Here is a great tool to use when writing smart goals. Can serve as a running record for teachers to have and use for student-teacher check ins.   Click on the icon to the right to access the google form, be sure to make a copy.

Once a week, I started giving my kids mini-checkpoints that informed me if my students had learned the weekly lessons.  This served as a quick form of assessment for me and for the students.  I used similar problems from the week's lessons and then used the Add-On called Flubaroo to keep their averages on a data sheet.  
If you don't want to deal with data sheets, just record your averages on your grade book or have students keep track of their scores.  
These mini-assessments were 3 problems, 1 point each.
